Authorization and Waiver

I authorize all previous employers and/or references to disclose to Dennen Steel Corp. any and all information in their possession about my employment history, including disciplinary and other matters. I herby waive written or other notice from such prior employers and/or references of their release of any such information to Dennen Steel Corp. I further authorize the education institutions listed in the application of employment to disclose to Dennen Steel Corp. any and all information in their possession regarding my attendance and performance at such institution, including but not limited to: disclosure of any diploma, degree of certification awarded; disclosure of any disciplinary record. I hereby waive written or other notice from such institution of its release of any such information to Dennen Steel Corp.

I affirm that all of the information contained on this job application and/or resume’ is true and complete and that any falsification, misrepresentation or omission herein may result in refusal of, or immediate dismissal from employment.

For applicants who receive an offer of employment, a pre-employment drug-screen is required. In the event of my employment, I will comply with all formal policies and communications directed to employees. I understand and agree that my employment can be terminated with or without cause and with or without notice at any time at the option of either me or the company. I understand that no employee of the company has the authority to enter into any agreement for employment for any specified period of time or to make any agreement contrary to the foregoing.

Dennen Steel is a one language company and all employees must be able to read, write and understand English.

I agree that I will not commence any action or suit related to my employment with the company (or termination of employment0 more than 180 days after the employment action at issue, and I agree to waive any statute of limitations to the contrary. I understand that this means that even if the law would give me the right to wait a longer time to make a claim, I am waiving the right, and that any claims not brought within 180 days of the action complained of will be barred.

For purposes of this Authorization and Waiver, selecting YES below shall have the same force and effect as my original signature.
